Your First TMS Session: What to Expect

Thinking about trying transcranial magnetic stimulation (TMS) therapy? Whether you’re exploring it for depression, anxiety or other mental health conditions, you’re probably wondering what the experience will be like.

Being concerned before any procedure is perfectly normal. Although TMS has been used successfully for years, it’s still a relatively new practice. This blog will walk you through your first TMS session, letting you know what to expect, including how long it lasts and what it generally feels like.

What is TMS?

TMS, or transcranial magnetic stimulation, is an evidence-based treatment for depression and other conditions. Non-invasive and drug-free, TMS is often used when other, more traditional means of treating depression haven’t worked.

TMS works by placing an electromagnet on your scalp. The magnetic fields generated by the electromagnet help stimulate areas of the brain involved in mood regulation and other functions. In turn, this seems to help treat the symptoms of depression and other mental health disorders.

The conditions TMS can be used for include:

Let’s break down what to expect during your first TMS session so you can walk in feeling confident and prepared.

Your First TMS Session: Getting to Know You

Your first TMS session is all about setting you up for success, and comfort is key. You’ll be seated in a comfortable chair (think: a cozy, reclined position) in a quiet and calming space. These chairs are highly adjustable, and your clinician will help you find the position you’re most comfortable in.

This is your time to relax and focus on your mental well-being. You’ll be in this position for a while (your first session usually takes more time), so wear something that’s going to be comfortable.

Your First TMS Session: Setting Up

In general, each TMS session lasts between 20 to 40 minutes. Your first TMS session may last a little longer as there’s extra time needed for a procedure called brain mapping.  Brain mapping takes around half an hour, and we only need to do it once.

To be effective, TMS has to be highly personalized. Brain mapping helps us determine the ideal treatment location and the amount and strength of pulses we use. We do this by moving the TMS electromagnet gently around your head. When the right area is found, your right hand’s thumb and fingers will move slightly.

After brain mapping and during the actual TMS session, you’ll be awake and alert, with no sedation involved. You can even use the time to relax, listen to music, binge your favorite series, or scroll through your phone.

Many people find the sessions quite relaxing. There’s no downtime afterward. You can resume your normal activities immediately.

Your First TMS Session: What It Will Feel Like

So, what does your TMS session actually feel like? While there are definitely physical sensations, it’s not as intense as you might think.

You may feel a mild tapping or tingling sensation on your scalp during the treatment, but it’s generally very tolerable. Some people describe it as similar to a light tapping or pulsing feeling on the skin.

The most important thing to know is that it’s not painful, and any discomfort tends to go away with repeated sessions.

Your First TMS Session: After Treatment

Once your session is over, there’s no recovery time needed. This non-invasive procedure doesn’t use anesthesia, so you can go about your day as usual. Some people may feel a little light-headed, headachy, or tired after the first few sessions, but this typically subsides as you continue with treatment.

If you have any concerns, your clinician will be there to support you.
